To dismiss transference this way is to misunderstand it. Even in the most rigid of places (traditional psychoanalytic literature) not everything can be dismissed as transference and it's simply not an accurate description of transference to say "the client isn't really talking to me but to her mother". I would honestly expect a prominent figure such as Spinelli to have a more nuanced and accurate understanding of transference even (and especially) if he aims to reject it as a relevant concept.

I think the quote about emotional memory converts the past into an expectation of the future without our conscious awareness could be better captured by changing the word converts to constructs. Convert means change but it is not changing it, it is constructing a narrative that makes sense to use as a rule or theory for what to expect in the future(in an unconscious way).

How does a person find a therapist with this level of knowledge and ability? It's impossible to find one, in my experience. I did EMDR over years and it was interesting and even wild at times but nothing changed at all. I'm certain it's because it's my stuff but i didn't have a therapist who could see that and work on it in the ways described in this interview. I just ordered Mr Ecker's book to study.

The more I learn about existential therapy, the more I see it to be the best way to proceed, for it addresses overarching paradigms and perennial problems that consistently plague people, whereas other forms attempt to treat the side effects of those paradigms.

Wonderful interview. Thank you very much for sharing it. In my country (Colombia), the topic of case formulation has been dominated by psychologists with a strictly behavioral approach. In my opinion, this has made it unnecessarily complicated, lacking in parsimony, and explained in a rather obscure and pretentious manner. This is what I love about American psychologists and scientists-they tend to be more parsimonious and practical. The conceptualization presented at minute 34:29 is simple, beautiful, yet also comprehensive. I hope to use Dr. Tracy's work to influence a different trend in my country regarding case conceptualization. One that is evidence-based but also parsimonious and practical.

... the client goes into destabilization. However, it is not a question that past schema is true while believing the new present schema. Both can be true. The past was true for the past. While the present for the now. The realization lies in that thou both true, yet cannot exist together.

The best decisions are evidence-based. Not experience. Not gut feelings, but real, hard evidence. In so many aspects of our lives, we think nothing of expecting others to base their decisions on evidence. When you take your car into the shop, do you want your mechanic making decisions on how to repair your automobile based upon his gut feelings? Or do you want your stock broker making decisions about your financial investments based upon her/his gut feelings? Most people want such decisions to be evidence-based. For psychotherapy, this means using the best research findings to make therapeutic decisions.

Yeah, the therapeutic fallacy like the gamblers fallacy or the drinkers fallacy or any fallacy; it’s fallacious…!!!! 😅 The answer is there is no answer…watch “Life of Brian” the bit where Brian is visited by “the people”. They camp on his doorstep & his mother, a Python in drag, tells them to “bugger off”…Brian then appeals to them observing that they are “all individuals duals” a lone voice, with perfect timing, opines…”I’m not!” This is therapy…there are no constructs that can be used other than “there are no constructs that can be carried into the inter subjective space”. Therapy is a construct created in the inter subjective space between client & therapist, unique to every session & as such cannot be anticipated except that all knowledge is no knowledge. As Jung said “leave it all at the door”…how can this man BOT have anticipated Alex’s question…”so what makes an expert?” He responded as if that question had never occurred to him…?!😮🤷‍♂️ That should have been his opening consideration, surely…? As a philosophy graduate from Corsica said to me outside a café in Malia in Crete; “…the point is there is no point…” Touché, absolutalimon….😊👍

The world of psychotherapy is continuously evolving, with new trends, techniques, and technologies shaping the way therapists interact with their clients and measure their progress. Among these trends, the shift towards evidence-based practice has become particularly significant. This trend is not just a fad; it’s a profound transformation that promises to make therapy more effective, efficient, and transparent.

Psychotherapy, a journey of introspection and healing, demands both the therapist and client to continuously track and understand progress. The Outcome Rating Scale (ORS) in psychology, with its brevity and simplicity, is a tool that offers significant benefits in this journey of emotional and mental discovery.

Yalom shows contempt for patients in his books. The therapy industry loves holding up individuals as masters because they wrote a book and the weird culture of having leaders grows. If only pscyhotherapy was actually as safe and effective as theyd' have you believe. Psychotherapy outcomes are not always positive. Approximately 40-60% of patients do not reach a recovery criterion (Fisher and Durham, 1999; Gyani et al., 2013; HSCIS, 2018) and between 5 and 8.2% have a negative outcome, with worse mental health at the end of therapy than at intake (Barkham et al., 2001; Hansen et al., 2002). Estimates vary because of measurement and population differences. However, there is an important difference between an unsuccessful therapy and a harmful one. Clinical deterioration can be caused by many factors external to the therapy, and failure to benefit from therapy does not imply harm. Negative effects of therapy are common, may be short-lived, and emotionally distressing experience may be an intrinsic part of good therapy (Schermuly-Haupt et al., 2018). Rozental et al. (2019) found that 50.9% of 564 clients in low intensity CBT reported some degree of adverse experience during therapy on the Negative Effects Questionnaire (NEQ). In contrast, in a survey of 14,587 British patients receiving National Health Service psychotherapy, 5% reported “lasting bad effects” of therapy (Crawford et al., 2016). Although this is a much smaller proportion, it represents a large number of patients who report that therapy has been, to some extent, harmful.
